Our Services

Extern Outreach Support Service (EOSS)

In partnership with the HSE’s Disability Services, the EOSS  service support children, young people and adults with an intellectual disability or autism and who may find it challenging to integrate into traditional social, educational or community systems.

EOSS is aligned with the HSE’s New Directions Principles, which include important elements such as person-centredness, community inclusion and active citizenship.

48 people were directly supported by our EOSS programme in 2022

This is achieved through a combination of individual outreach work, integration into community networks, special interest activities, day trips, respite and day care support.

Through EOSS we:
  • Offer a needs-led, creative, inclusive and flexible approach to service user needs.
  • Support service users to be active independent members of their own communities.
  • Develop and implement bespoke independent living skills programmes.
  • Respond to behaviours of concern in line with a positive behaviour support model.
  • Support individuals to develop positive coping strategies during times of crisis.
  • Provide the opportunity for individuals to engage in activities and time away from home to promote health and wellbeing and maintain sustainable home placements.
