Multi-Disciplinary Homeless Support Team
Extern's Multi-Disciplinary Homeless Support Team provides assistance, support and advocacy to adults and families who are homeless or at risk of homelessness and have complex additional needs.
In the past year, our Multi-Disciplinary Homeless Support Team has made an incredible impact, offering crucial support to 737 individuals experiencing homelessness.
Offering a comprehensive floating support service, our team works to address these multifaceted needs.
In collaboration with the Northern Ireland Housing Executive and Belfast Health & Social Care Trust, our service has significantly expanded its reach since 2018.
Our presence now extends into the South Eastern Trust area, amplifying our service delivery from Belfast.
Additionally, we've established support initiatives across dispersed projects in North Down and Ards, Mid-Ulster, and Enniskillen. Referrals for these dispersed projects are facilitated through a closed pathway via NIHE.
Accessing our services is straightforward: individuals can engage with MDHST through open or self-referral. For referrals pertaining to the South Eastern and dispersed projects, connections are established via the local NIHE office.
Our aim is to provide accessible and comprehensive support, meeting individuals at their point of need across various regions.
